Physicians
We know that many physicians like to participate in the development of their hospice patients' Plan of Care and to be kept informed of any changes in their patients' condition. We are a "physician-friendly" hospice and encourage referring physician participation in the care that we offer to your patients. We will actively communicate with you about your patients' condition using the communication protocols we establish with your office. Long-Term Care Providers
Hospice of Virginia's Long-Term Care "service promises" are custom designed to support your facility's ongoing efforts to provide the care that terminally ill residents need and are entitled to as part of their Medicare and Medicaid Benefits. We know how important it is for you to provide continuity of care for your residents while being fiscally responsible in your operations. We help you keep residents in your facility – through crisis care when appropriate and with the supplemental resources provided under the Medicare Hospice Benefit – such as visits from members of the hospice team, as needed by the patients – any day of the week. We can also contract with your facility to provide the hospice inpatient level of care for patients that meet the criteria for that level of care – if your facility meets the regulatory requirements.
We can also help your facility achieve excellent survey results in all areas of end-of-life care – by taking advantage of programs we've developed and can share with you in the areas of pain management, advance directives, nutrition/hydration, assuring accessibility of the hospice team when needed by patients and families; whether during the day, evenings or weekends. We invite you to work with a hospice partner who understands your business and has learned to work within your care culture. Our staff always act as "guests" in your facility.
Hospital Providers
At Hospice of Virginia we know how important it is for hospital discharge planning and utilization review staff to free up needed hospital beds. Further, the discharge of a patient to hospice services is stressful for hospital staff, patients, and family members. Our promise is to provide immediate response to your patient referrals. Our goal is to make contact with the patient and family within the hour of your call to our referral center and to assess the appropriateness of each referral and admit appropriate patients within three (3) hours on the day of referral.
We know that a smooth transition from hospital to home (or other residential setting) is your commitment to your patients. Our admission staff are exceptional at anticipating what is required for a smooth transition for each patient that we accept into our program of care. We will ensure that all aspects of the transition will be coordinated so that the patient is comfortably transferred with the necessary medications, equipment, and medical supplies. We will also ensure that family members and caregivers are prepared for what will take place during the transition and when things will occur.
